Candidates fulfilling the eligibility criteria will have to submit these documents along with the filled application and 6 passport size recent photographs.

1. S.S.L.C. Marks Card
2. PUC Marks Card
3. Transfer Certificate
4. Migration Certificate (For non-karnataka domiciliary candidates)

1. 4 years BPT Marks cards
2. Internship Completion certificate
3. Transfer Certificate
4. Degree Certificate
5. Migration Certificate (For candidates completed BPT in universities other than RGUHS
